What it does
A visitor fills out a form; your team gets the complete record — the answers, the files, and a finished PDF. It lives in three places:
Build forms in a visual editor inside Shopify admin — sections, fields, and rules — and review every submission in one list.
Each form gets its own web address, and drops into any page with the Form embed theme block — no code either way.
Every submission is emailed to the recipients you choose, with uploaded files attached and, optionally, the filled PDF.

Why we built it
Existing form apps stop at collecting the data and notifying you. The businesses we work with needed the next step — each submission as a finished, branded document, delivered to the person who acts on it. So we built Professional Form Manager.
